Job summary

UnitedHealth Group is seeking an ECF CHOICES Support Coordinator to manage a panel of members with intellectual or developmental disabilities. This home-based role includes 75% field work and remote flexibility within a 50-mile radius of Williamson County, TN.

You will assess medical, behavioral, functional, cultural, and social needs, develop person-centered care plans, and coordinate with providers to improve health outcomes and quality of life for members.

Qualifications

• Meet one of the following: 4-year or higher degree in social work, nursing, education, rehabilitation counseling, or other human service or health care profession.
• Be an RN or LPN.
• Meet the federal requirements for a QDDP or QIDP.
• 5+ years of independent Support Coordinator or case manager experience in a 1915(c) HCBS Waiver.

Responsibilities

• Engage members face-to-face or by phone to complete comprehensive needs assessments.
• Develop and implement person-centered care plans addressing medical, behavioral, and social needs.
• Coordinate with internal care team and community resources to execute care plans.
• Provide referrals and linkages to housing, pharmacy, peer support, and other services.
• Collaborate with providers across settings to support member goals.
