Job summary

UnitedHealthcare is seeking a Field Care Coordinator in Tennessee, primarily home-based with substantial field responsibilities. You will assess medical, behavioral, functional, and social needs and develop person-centered care plans to improve health outcomes.

The role requires a 4-year degree in a relevant field or RN/LPN licensure, or QDDP/QIDP credential, plus 5+ years in a similar coordination role. Travel up to 75% to meet members and providers is expected.

Qualifications

• Bachelor's degree or RN/LPN, or QDDP/QIDP eligibility.
• 5+ years of independent Support Coordinator or case manager experience in 1915(c) HCBS Waiver.
• Ability to travel up to 75% within Tennessee counties and surrounding areas.

Responsibilities

• Engage members face-to-face and/or telephonically to complete comprehensive needs assessments including medical, behavioral, functional, cultural, and socioeconomic needs.
• Develop and implement person-centered care plans addressing chronic conditions, wellness, SDoH, medication management, and safety.
• Collaborate with internal care teams, providers, and community resources to execute care plans.
• Provide referrals and linkage to housing, pharmacy, peer supports, and other services as appropriate.
