Job summary

UnitedHealth Group is seeking a Support Coordinator for a panel of members with intellectual or developmental disabilities. The role emphasizes comprehensive needs assessment and person-centered care planning to improve health outcomes.

The position is home-based with field responsibilities about 75% of the time and requires travel within Tennessee counties to meet members and providers. Telephonic and in-person interactions are expected to deliver coordinated care.

Qualifications

• RN or LPN licensure or equivalent clinical credential.
• 4-year or higher degree in social work, nursing, education, rehabilitation counseling, or health care/profession related to human services.
• Meet federal requirements for QDDP or QIDP.

Responsibilities

• Complete comprehensive needs assessments for members (medical, behavioral, functional, cultural, SDoH).
• Develop and implement person-centered care plans addressing chronic conditions, health promotion, SDoH, and safety.
• Collaborate with care teams, providers, and community resources to execute care plans.
• Provide referrals and linkages to internal or community services as appropriate.

Education
