Job summary

UnitedHealthcare in Tennessee is seeking a Field Care Coordinator for Williamson County to support members in the Employment and Community First CHOICES program. You will conduct needs assessments, develop person-centered care plans, and coordinate with providers and community resources to improve care and outcomes.

The role is home-based with about 75% field work and requires travel within the county and surrounding areas.

Qualifications

• Meet one of the following: 4-year or higher degree in social work, nursing, education, rehabilitation counseling, or other human service (e.g. psychology, sociology) or health care profession
• Be an RN or LPN
• Meet the federal requirements for a QDDP or QIDP
• 5+ years of independent Support Coordinator or case manager experience in a 1915(c) HCBS Waiver and have completed Personal Outcome Measures Introduction and Assessment Workshop trainings

Responsibilities

• Engage members face-to-face and/or telephonically to complete comprehensive needs assessments
• Develop and implement person-centered care plans addressing medical, behavioral, functional, cultural, and socioeconomic needs
• Coordinate with internal care team, providers, and community resources to implement care plans
• Provide referrals and linkage to housing, pharmacy, peer specialists, PCPs and other providers
• Work within commutable distance to Williamson County and maintain travel as needed
