Job summary

UnitedHealth Group is seeking a Support Coordinator to lead member-focused care for individuals with intellectual or developmental disabilities. This home-based role involves field work about 75% of the time, coordinating medical, behavioral, and social needs to improve care utilization and outcomes.

You will assess needs, create person-centered care plans, and collaborate with care teams and community resources to provide effective referrals and support services.

Qualifications

• Must be an RN or LPN or hold a relevant human services degree.
• Experience as an independent Support Coordinator or case manager is required.
• Must reside within 50 miles of TN counties or commutable distance and have a valid US driver’s license.
• Travel up to 75% within advertised counties and surrounding areas.

Responsibilities

• Perform comprehensive needs assessments covering medical, behavioral, functional, cultural, and socioeconomic needs.
• Develop and implement person-centered care plans addressing chronic conditions and social determinants of health.
• Coordinate with internal teams and external providers to execute care plans.
• Provide referrals to Housing Navigator, Pharmacy, Peer Specialist, and community resources as appropriate.
• Engage with members face-to-face or by telecommunication to promote quality of care.
