Job summary

UnitedHealthcare is seeking an ECF CHOICES Support Coordinator to manage a panel of members with intellectual or developmental disabilities, focusing on medical, behavioral, and socioeconomic needs to improve quality of care. This is a home-based role with field responsibilities about 75% of the time.

If you reside within a 50 mile radius of Dixon, Montgomery, and West Davidson Counties in Tennessee, you may work remotely while visiting members and providers as needed.

Qualifications

• 4-year or higher degree in social work, nursing, education, rehabilitation counseling, or other human service or health care profession.
• RN or LPN licensure.
• QDDP or QIDP certification (federal requirements).
• 5+ years of experience as a support coordinator or case manager in HCBS Waiver settings.

Responsibilities

• Engage members in person or by telecommunication to complete comprehensive needs assessments.
• Develop and implement person-centered care plans addressing medical, behavioral, and social needs.
• Collaborate with care teams, providers, and community resources to implement plans.
• Provide referrals and linkages to Housing Navigator, Pharmacy Team, Peer Specialists, and others as appropriate.
• Travel up to 75% within Tennessee to meet with members and providers.
