Job summary

UnitedHealth Group is hiring a Care Coordinator to manage care for dual eligible members across medical, behavioral, and social needs. The role supports a person-centered approach, coordinating with providers and resources to improve outcomes and reduce unnecessary utilization.

Remote-friendly, flexible scheduling, and a focus on equity and quality of care. Strong organizational skills and 1+ years in Medicaid/Medicare or social determinants of health are preferred.

Qualifications

• Current Idaho license: LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT or LPN/LVN.

Responsibilities

• Serve as primary care manager for dual eligible members and develop individualized, person-centered care plans.
• Collaborate with internal care team and community resources to implement plans and remove obstacles.
• Coordinate care transitions and discharge planning after ER/inpatient/SNF stays.
• Provide education and coaching to support self-management and healthy lifestyle choices.
• Advocate for members’ needs and guide nonclinical questions to appropriate resources.
