Job summary

UnitedHealth Group is hiring a Field Care Coordinator to manage a panel of members with complex needs, focusing on medical, behavioral, and socioeconomic supports. This is a full-time, field-based role with a home office, including remote work within NM areas like Albuquerque, Santa Fe, or Roswell.

You will assess needs, create care plans, coordinate with providers, and help members navigate health and social services while promoting self-management and health literacy.

Qualifications

• Bachelor's degree or 2+ years of healthcare experience.
• Current, unrestricted RN or LCSW/LMSW/LMHC licensure with 3+ years clinical.
• 5+ years of relevant experience, including 3+ years clinical.
• 4+ years in a clinical environment (1+ year as Care Coordinator).
• LPN with 3+ years clinical experience.
• Driver's license and ability to travel ~50% within territory.
• Must reside in Roswell, NM or commutable distance.

Responsibilities

• Engage members to complete comprehensive needs assessments.
• Develop and implement person-centered care plans.
• Collaborate with care team, providers, and community resources.
• Provide education to support self-management of care needs.
• Coordinate Care Transitions post-ER or SNF admissions.
• Advocate for member needs within the care team.
