Job summary

UnitedHealthcare is seeking a dedicated ECF CHOICES Support Coordinator to manage a panel of members with intellectual or developmental disabilities. You will assess needs, develop plans, and coordinate care with providers while traveling within TN counties.

The role emphasizes person-centered care, collaboration with care teams, and linkage to community resources, with flexible remote work and significant field time in Tennessee.

Qualifications

• 4-year or higher degree in social work, nursing, education, rehabilitation counseling, or other human service or health care profession.
• RN or LPN license.
• Federal requirements for a QDDP or QIDP certification.
• 5+ years as independent Support Coordinator or case manager for 1915(c) HCBS Waiver with related training.

Responsibilities

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment.
• Develop and implement person-centered care plans for medical, behavioral, and SDoH needs.
• Create individualized care plans aligned with readiness to change for health outcomes.
• Partner with internal care teams, providers and community resources to implement care plans.
• Provide referral and linkage to housing navigator, pharmacy team, peer specialist, and providers.
