Job summary

UnitedHealth Group is seeking a Support Coordinator to manage a panel of members with intellectual or developmental disabilities. You will perform comprehensive needs assessments and develop person-centered care plans to improve health outcomes.

The role is home-based with about 75% field work. Candidates must meet RN/LPN qualifications or have 5+ years as a HCBS coordinator, with potential QDDP/QIDP eligibility and travel up to 75% within TN counties.

Qualifications

• Be an RN or LPN or have 5+ years as an independent Support Coordinator or case manager in HCBS.
• A 4-year or higher degree in a human service or health field.
• Meet federal requirements for QDDP or QIDP.
• Reside within 50 miles of listed Tennessee counties.

Responsibilities

• Complete comprehensive needs assessments including medical, behavioral, functional, cultural, and SDoH needs.
• Develop and implement person-centered care plans addressing chronic conditions and wellness.
• Coordinate with care team, providers, and community resources to implement care plans.
• Provide referrals and linkage to Housing Navigator, Pharmacy Team, Peer Specialist, etc.
• Telecommute options with 75% field responsibilities and travel as needed.
