Job summary

UnitedHealth Group is seeking a Support Coordinator to manage a panel of members with intellectual or developmental disabilities. You will perform comprehensive needs assessments, develop person-centered care plans, and coordinate with care teams to improve health outcomes.

This is a home-based role with about 75% field responsibilities and remote work flexibility for local Tennessee regions. The role requires clinical credentials (RN/LPN) or relevant experience, a strong background in HCBS, and

Qualifications

• Meet one of the following: RN or LPN
• 5+ years as an independent Support Coordinator/case manager for 1915(c) HCBS Waiver
• 4-year or higher degree in social work, nursing, education, rehabilitation counseling, or other human service
• Meet the federal requirements for a QDDP or QIDP
• Reside within 50-mile radius of Sevier & Knoxville County, TN
• Access to reliable transportation and valid US driver’s license with travel up to 75%

Responsibilities

• Engage members face-to-face or telephonically for comprehensive needs assessment
• Develop and implement person-centered care plans
• Collaborate with internal care team, providers, and community resources
• Provide referrals and linkage to appropriate services
