Job summary

UnitedHealth Group is seeking a Care Coordinator in Idaho to serve as the primary care manager for a panel of dual-eligible members, focusing on medical, behavioral, functional, and social drivers of health. The role supports person-centered care coordination, collaboration with care teams, and linkage to community resources.

You will engage members in assessments, develop individualized care plans, and coordinate transitions across care settings.

Qualifications

• 1+ years of experience with Medicaid/Medicare or social drivers of health (SDoH) needs.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Driver's license and ability to travel within territory up to 50% of the time.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs or wellness assessments and develop care plans.
• Collaborate with internal care teams, providers, and community resources to implement plans.
• Assist members with HCBS supports and services as needed.
• Provide referrals and linkage to relevant internal or community resources.
