Job summary

This role focuses on person-centered care, coordinating medical, behavioral, and social needs, and partnering with internal teams and community resources.

Location in Idaho with remote work flexibility. Must have an Idaho license (LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT or LPN/LVN) or equivalent, and 1+ year Medicaid/Medicare experience.

Qualifications

• 1+ years of experience with Medicaid/Medicare or strong social determinants of health experience.
• 1+ years of experience with MS Office (Word, Excel, Outlook).
• Access to a quiet home workspace with ability to protect PHI.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs/wellness assessments and develop person-centered care plans.
• Collaborate with care teams and community resources to implement plans.
• Assist members in obtaining HCBS support and services.
• Provide referrals to appropriate internal/external resources.
• Support discharge planning and care transitions after ER or facility stays.
• Educate and coach members for self-management and healthy lifestyle changes.
• Advocate for members to ensure their needs are supported by the care team.
• Coordinate nonclinical inquiries with providers and Health Plan resources.
