Job summary

UnitedHealthcare is seeking a Care Coordinator to be the primary care manager for a panel of members with chronic and complex health care needs in Idaho. The role supports medical, behavioral, and social drivers of health to promote appropriate utilization and improved outcomes.

Remote-friendly and fast-paced, the position requires collaboration with internal teams and community partners, strong organizational skills, and a commitment to person-centered care delivery.

Qualifications

• Must hold one of the listed Idaho licenses or be eligible.
• 2-year degree (or higher) and 2+ years healthcare experience.
• 1+ years Medicaid/Medicare or SDoH experience.
• 1+ years MS Office experience.
• Quiet home workspace with PHI security.
• Reliable internet and ability to travel up to 50%.

Responsibilities

• Serve as primary care manager for dual eligible members.
• Perform comprehensive needs/wellness assessments and create person-centered care plans.
• Collaborate with care team, providers and community resources to execute plans.
• Assist members with HCBS supports and services.
• Provide referrals to internal or community providers as appropriate.
• Support discharge planning and care transitions after ER/inpatient stays.
• Educate and coach for self-management of health and lifestyle changes.
• Advocate for member needs within the health care team.
• Handle nonclinical questions and connect to Health Plan resources.
