Qualifications

• Proficient in MS Office (Word/Excel/Outlook) and able to use EHR systems.

Responsibilities

• Serve as primary care manager for dual-eligible members and conduct comprehensive needs assessments.
• Collaborate with care team and community resources to implement person-centered care plans.
• Assist with discharge planning and care transitions to support community-based living.
