Job summary

UnitedHealthcare is seeking a Care Coordinator to serve as primary care manager for a panel of members with chronic/complex health needs. You will support medical, behavioral, and socioeconomic needs, coordinating with internal teams and community resources.

The role requires strong organizational skills and the ability to multitask in a fast-paced environment. Remote work option available for Idaho Falls, ID or surrounding areas with up to 50% travel within territory.

Qualifications

• Current and unrestricted Idaho license in LCSW, LMSW, LSW, LCPC, LPC, LMFT, or LAMFT
• LPN/LVN license
• 2-year degree with 2+ years healthcare experience
• 1+ years experience with Medicaid/Medicare or SDoH needs
• 1+ years MS Office experience
• Access to a designated quiet home workspace with PHI security
• Residence in an area with UnitedHealth Group approved internet
• Driver's license and ability to travel up to 50%

Responsibilities

• Serve as primary care manager for dual eligible members
• Conduct comprehensive needs or wellness assessments and implement person-centered care plans
• Collaborate with care team, providers, and community resources to execute care plans
• Assist members with HCBS supports and services
• Provide referrals to internal and external resources
• Support discharge planning and care transitions after ER or facility stays
• Educate and coach members on self-management and healthy lifestyle changes
• Advocate for members to ensure needs are represented
• Coordinate nonclinical questions with providers and health plan resources

Education

Idaho license (LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT) or LPN/LVN

2-year degree + 2+ years healthcare experience

1+ years Medicaid/Medicare or SDoH experience

MS Office (Word, Excel, Outlook)

PHI-secure home workspace

Driver's license & 50% travel
