Job summary

UnitedHealthcare is seeking a Care Coordinator to act as the primary care manager for a panel of members with chronic and complex health care needs. You will support medical, behavioral, and social drivers of health and collaborate with a broad care team to implement person-centered plans.

This role offers remote flexibility for candidates in Idaho, with opportunities to engage with providers and community resources to improve care and reduce barriers to access.

Qualifications

• Current and unrestricted Idaho license in LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT or LPN/LVN
• 1+ years experience with Medicaid/Medicare or SDoH
• 1+ years experience with MS Office (Word, Excel, Outlook)
• Access to a designated quiet workspace with ability to secure PHI
• Residence in a location with UnitedHealth Group approved high-speed internet
• Reliable transportation and valid U.S. driver's license with travel up to 50% of time

Responsibilities

• Serve as the primary care manager for dual eligible members
• Conduct comprehensive needs/wellness assessments and design person-centered care plans
• Collaborate with internal care team and community resources to implement plans
• Assist members with HCBS support and services
• Provide referrals and linkages to internal and external resources
• Support discharge planning and care transitions following ER visits
• Educate and coach members on self-management and lifestyle changes
• Advocate for members to ensure needs are represented
• Support nonclinical queries from providers and facilities, coordinating with Health Plan/UnitedHealthcare resources
