Job summary

UnitedHealth Group is seeking a Care Coordinator in Idaho to lead care management for dual eligible members with chronic and complex health needs.

You will collaborate with the internal care team, providers and community resources to implement personalized care plans while supporting member self-management and transitions across care settings. Remote work flexibility is available within a fast-paced, multitasking environment.

Qualifications

• Current Idaho license in a clinical/social work field or RN/LPN license.
• 1+ years with Medicaid/Medicare or SDoH needs.
• Proficient in MS Office (Word, Excel, Outlook).
• Must have valid driver’s license and access to reliable transportation.

Responsibilities

• Serve as primary care manager for dual eligible members.
• Conduct comprehensive needs and wellness assessments and develop care plans.
• Collaborate with care team, providers and community resources to implement plans.
• Assist members with HCBS supports and referrals as needed.
• Coordinate transitions from ER, inpatient, or SNF to home/community.
• Educate and coach members for self-management and healthy lifestyle changes.
• Advocate for member needs within the health care team.
