Job summary

UnitedHealthcare is seeking a Care Coordinator to serve as the primary care manager for a panel of dual-eligible members with chronic and complex health care needs. You will conduct comprehensive assessments and develop person-centered care plans, collaborating with the internal team and community resources to remove barriers and promote quality care.

The role supports care coordination across medical, behavioral, and social drivers of health, with remote work options in Idaho and defined travel

Qualifications

• Idaho license options must be current and unrestricted
• 1+ years experience with Medicaid/Medicare or SDoH
• 1+ years MS Office experience
• Dedicated workspace at home with PHI security
• Reliable high-speed internet in Idaho
• Valid US driver's license and 50% travel as needed

Responsibilities

• Serve as primary care manager for dual eligible members
• Conduct comprehensive needs/wellness assessments and develop person-centered care plans
• Collaborate with internal care team and community resources to implement plans
• Assist members with HCBS supports and services
• Provide referrals to internal/external resources
• Support discharge planning and care transitions
• Educate and coach for member self-management
• Advocate for member needs
• Connect providers to Health Plan resources as needed

Face-to-face & telephonic engagement

Education

Idaho professional license options: LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT; or LPN/LVN; or 2-year degree + 2+ years healthcare experience
