Job summary

UnitedHealth Group is hiring a Care Coordinator in Meridian, ID to serve as the primary care manager for a panel of members with chronic and complex health needs. You will coordinate care, collaborate with the internal team and community resources, and support member self-management and transition needs.

Ideal candidates hold an Idaho license (LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT or LPN/LVN), have Medicaid/MediCare experience, and can travel up to 50% while working in a remote-friendly environment

Qualifications

• Current and unrestricted Idaho license in one of the listed credentials.
• 1+ years of experience with Medicaid/Medicare or SDoH-driven care needs.
• 1+ years of experience with MS Office (Word/Excel/Outlook).
• Driver's license and ability to travel up to 50% of time.
• Home workspace that is PHI-secure and a high-speed internet setup.

Responsibilities

• Serve as primary care manager for dual eligible members.
• Conduct comprehensive needs or wellness assessments and develop person-centered care plans.
• Collaborate with care teams, providers, and community resources to enable plan execution.
• Assist members in obtaining HCBS supports and services.
• Provide referrals and linkages to internal and external resources as appropriate.
• Support discharge planning and coordinate care transitions from ER or SNF.
• Educate and coach members for self-management and lifestyle changes.
• Advocate for member needs within the health care team.
