Job summary

UnitedHealthcare is seeking a Care Coordinator to manage a panel of members with chronic and complex health needs. The role focuses on person-centered care, coordinating medical, behavioral, and social drivers of health to improve outcomes.

The position supports a flexible work arrangement for residents in Twin Falls, ID or surrounding areas, with some remote work. Candidates should have relevant licenses/certifications and at least 1 year of healthcare and MS Office experience.

Qualifications

• Must have a current Idaho license option (LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT) or LPN/LVN or a 2-year degree with 2+ years healthcare experience
• 1+ year of Medicaid/Medicare or SDoH experience
• Experience with MS Office (Word, Excel, Outlook)
• Home workspace with ability to secure PHI
• Access to reliable internet in assigned territory with up to 50% travel as needed

Responsibilities

• Serve as the primary care manager for dual eligible members
• Conduct comprehensive needs or wellness assessments and develop person-centered care plans
• Collaborate with care team, providers and community resources to implement care plans
• Assist with obtaining HCBS supports and services
• Provide referrals to appropriate internal or external resources
• Support discharge planning and care transitions after ER visits
• Educate and coach members for self-management of care
• Advocate for patient needs within the health care team
• Answer nonclinical questions and connect providers to appropriate resources
