Job summary

Sentara Health is seeking an Epic Healthy Planet Senior IT Specialty Analyst for a fully remote role. You will support and optimize Epic Healthy Planet care management workflows, collaborate with care managers and leadership, and drive scalable solutions across the care continuum.

The position requires Epic Healthy Planet Certification with 5–7 years of related experience, and familiarity with EpicCare/ Ambulatory tools like Reporting Workbench and SlicerDicer.

Qualifications

• Epic Healthy Planet Certification is required.
• Experience with Epic Care Ambulatory, Reporting Workbench, and SlicerDicer is important.
• familiarity with CMS quality programs, HEDIS measures, and risk adjustment is expected.
• 5 to 7 years of relevant experience is required.

Responsibilities

• Day-to-day support, build and optimization of Epic Healthy Planet care management and care coordination workflows and configuration.
• Partner with care managers, clinical and operations leadership and informatics teams to implement standard, scalable solutions.
• Develop test plans, handle incident coverage and enhancement requests, and support quarterly Epic updates.
