Job summary

Tuesday Health in Warren, Ohio is seeking a Licensed Practical Nurse to join our Complex Care Navigator team, delivering coordinated, compassionate care for members with serious illness. You will work with nurse practitioners and RNs to assess, prioritize needs, and develop care plans using our electronic systems.

The role emphasizes empathy, clear communication, and member engagement, with driving required and remote work options paired with on-site sessions to maximize collaboration and care

Qualifications

• Active and unrestricted LPN license in the State of Ohio with no board action.
• Experience in a clinical/medical setting preferred.
• Experience in a multi-disciplinary care model.
• Strong communication and organizational skills.
• Ability to multitask and work under pressure while maintaining patient experience.

Responsibilities

• Administer multiple assessments and screening tools within the clinical model with support from providers.
• Prioritize patient needs based on assessments and coordinate with the care team.
• Be accountable for care plan development within the multidisciplinary team.
• Participate in care rounds to ensure coordinated patient experience.
• Learn and use Tuesday Health electronic systems and tools to deliver coordinated care.
• Build trusting relationships with patients and families through listening and empathy.
• Serve as the quarterback of the multidisciplinary team to transform care for members.
