Qualifications

• Active and unrestricted RN license in Ohio.
• Experience in a clinical/medical setting preferred.
• Experience working in a multi-disciplinary care model.
• Strong communication and organizational skills.
• Ability to navigate diverse documentation systems and learn new tools.
• Strong written communication skills.
• Valid driver's license with safe driving record.
• State minimum automobile insurance coverage.

Responsibilities

• Administer and interpret assessments and screenings.
• Develop individualized care plans with the care team.
• Lead care coordination and participate in rounds.
• Build trusting relationships with members and drive a coordinated approach.
• Learn and use Tuesday Health electronic systems and tools.
• Ensure a technology-enabled, streamlined member experience.
• Serve as quarterback of the multidisciplinary team delivering complex care.
