Job summary

The Complex Care Navigator LPN at Tuesday Health partners with nurse practitioners and registered nurses to deliver coordinated, compassionate care for members with serious illnesses in the Dayton area. You will perform clinical assessments, contribute to care plans, and participate in rounds while using our electronic systems to optimize the member experience.

A strong candidate holds an active Ohio LPN license, clinical experience, and excellent communication skills, with willingness to travel

Qualifications

• Active LPN license in Ohio.
• Clinical/medical setting experience preferred.
• Experience in a multidisciplinary care model.
• Strong communication and organizational skills.
• Ability to multitask and work under pressure.

Responsibilities

• Administer assessments and screenings within the clinical model with support from NP/RN.
• Prioritize member needs and develop care plans within the team.
• Participate in rounds to ensure coordinated member experience.
• Learn and use Tuesday Health electronic systems to optimize care.
