Tuesday Health is seeking an experienced Complex Care Navigator RN to lead comprehensive, person-centered care for individuals with serious illness. You will administer assessments, develop individualized care plans, and drive care coordination using our electronic systems to ensure a streamlined, compassionate member experience.

This role requires an active OH RN license, strong communication, and the ability to navigate multiple systems.

Qualifications

• Active and unrestricted RN license in Ohio, no board actions.
• Experience in clinical/medical settings preferred.
• Experience in a multidisciplinary care model across providers.
• Strong communication and organization skills.
• Ability to multitask and manage pressure while maintaining patient experience.
• Know when to escalate and act professionally in emergent situations.
• Technical ability to navigate multiple documentation systems and learn new ones.
• Ability to drive during day/night or inclement weather.
• Valid driver’s license and minimum auto insurance.
• Comfort with change and rapid evolution of business.

Responsibilities

• Administer multiple assessments and screening tools at various points in the care timeline.
• Interpret assessment responses and prioritize needs accordingly.
• Own care plan development within the multidisciplinary team.
• Lead internal and joint rounds to optimize the member experience.
• Learn and use Tuesday Health electronic systems to deliver coordinated care.
• Build strong, trusting relationships with members through listening and empathy.
• Serve as quarterback of the multidisciplinary team to transform care for seriously ill members.
