Job summary

Tuesday Health in Warren/Youngstown, Ohio seeks a Licensed Practical Nurse to support care for members with serious illness. You will work with nurse practitioners and RNs delivering assessments and developing care plans.

You will participate in rounds, use electronic systems, communicate compassionately, and build trust with members. A valid Ohio LPN license and the ability to drive are required.

Qualifications

• Active and unrestricted LPN license in Ohio.
• Experience in clinical settings preferred.
• Experience in multidisciplinary care models is a plus.
• Strong communication and organizational skills.
• Ability to multitask and work under pressure.

Responsibilities

• Administer multiple assessments and screening tools in the clinical model with NP/RN support.
• Prioritize member needs based on assessments and coordinate with the care team.
• Be accountable for care plan development within the multidisciplinary team.
• Participate in rounds to ensure a coordinated member experience.
• Learn and use Tuesday Health electronic systems and tools.
• Build trusting relationships with members through empathy and listening.
• Act as quarterback of the multidisciplinary team for members with serious illness.
