Job summary

Tuesday Health is seeking a Licensed Practical Nurse to join our value-based palliative care team in Warren/Youngstown, OH. You will perform clinical assessments, collaborate with NPs and RNs, and help develop personalized care plans to improve quality of life for patients at home.

You will participate in rounds, utilize electronic systems, and build strong relationships with patients and families. A valid OH LPN license and strong communication skills are essential.

Qualifications

• Active and unrestricted LPN license in Ohio.
• Experience in clinical/medical settings preferred.
• Experience in multi-disciplinary care models.
• Strong communication and organizational skills.
• Ability to multitask while maintaining a positive member experience.
• Know when to escalate in emergent situations.
• Proficiency with documentation systems; ability to learn new systems.

Responsibilities

• Administer assessments and screening tools with support from nurse practitioners and RNs.
• Prioritize needs based on assessments and coordinate with the interdisciplinary care team.
• Be accountable for care plan development within the multidisciplinary team.
• Participate in rounds to ensure a coordinated member experience.
• Learn Tuesday Health electronic systems and tools to deliver coordinated care.
• Build trusting relationships with members through listening and empathy.
• Serve as quarterback of the multidisciplinary team delivering care for members with serious illness.
