Job summary

Tuesday Health seeks a Licensed Practical Nurse to deliver coordinated, compassionate support for members with serious illnesses in Ohio. The LPN will work with nurse practitioners and registered nurses to perform assessments, develop care plans, and ensure clear communication across the care team.

The role emphasizes empathy, adaptability, and a person-centered approach, with remote work and periodic onsite sessions to strengthen collaboration and member experience.

Qualifications

• Active and unrestricted LPN license in Ohio with no board action.
• Experience in clinical/medical settings preferred.
• Experience in multidisciplinary care model beneficial.
• Strong communication and organizational abilities.
• Ability to multitask and maintain a positive member experience.
• Willingness to drive as needed and adapt to changes.

Responsibilities

• Administer assessments and screenings with NP/RN support.
• Prioritize needs based on assessments and coordinate with care team.
• Develop and execute individualized care plans within the team.
• Participate in rounds to ensure a coordinated member experience.
• Learn and use Tuesday Health electronic systems and tools.
• Build trustful relationships with members through empathy.
• Serve as part of the multidisciplinary team driving care.
