Job summary
Tuesday Health is seeking a Licensed Practical Nurse to join our Complex Care Navigator team in Ohio. The LPN will perform clinical assessments, collaborate with NPs and RNs, participate in care rounds, and help develop individualized care plans using our electronic systems.
Strong communication and empathy are essential for a person-centered approach to care. The role supports a hybrid model with remote work options and onsite sessions, focusing on shaping compassionate care for seriously ill
Qualifications
- Active, unrestricted LPN license in Ohio.
- Clinical/medical setting experience preferred.
- Experience in a multi-disciplinary care model.
- Strong communication and organizational skills.
- Ability to drive with a valid license.
Responsibilities
- Administer assessments and screening tools within the clinical model with support from RNs/NPs.
- Prioritize needs based on assessments and coordinate with the care team.
- Develop and implement care plans within the multidisciplinary team.
- Participate in rounds to ensure coordinated member experience.
- Learn and use Tuesday Health electronic systems to support care delivery.
