Job summary

Tuesday Health is hiring a Complex Care Navigator RN in Ohio for a remote role with onsite sessions. You will lead care coordination, develop individualized plans, and participate in rounds to optimize the member experience. The ideal candidate holds an active Ohio RN license and has experience in clinical settings.

You'll use our electronic systems to deliver a coordinated, tech-enabled approach and build strong, empathetic relationships with members, driving collaboration across the care team.

Qualifications

• Active and unrestricted Registered Nurse license in Ohio.
• Experience in clinical/medical setting preferred.
• Strong communication and organization skills.
• Ability to multitask and work under pressure without sacrificing a positive member experience.

Responsibilities

• Administer assessments and screening tools within the clinical model across the care timeline.
• Interpret responses and support prioritization of need based on results.
• Be accountable for care plan development within the multidisciplinary care team.
• Lead in internal and joint rounds to ensure coordinated member experience.
• Learn and utilize Tuesday Health electronic systems to deliver coordinated care.
• Build trusting relationships with members through empathy and listening.
• Serve as the quarterback of the multidisciplinary team transforming care for members with serious illness.
