Tuesday Health, a value-based palliative care provider, seeks a Licensed Practical Nurse to join the Warren/Youngstown, Ohio team. You will work with nurse practitioners and RNs to assess, screen, and implement individualized care plans for seriously ill members, delivering compassionate, person-centered care.

Responsibilities include conducting assessments, coordinating with the multidisciplinary team, and ensuring seamless communication through our electronic systems.

Qualifikationen

• Active LPN license in Ohio.
• Clinical/medical setting experience preferred.
• Experience in multidisciplinary care model.
• Strong communication and organizational skills.
• Ability to learn new systems as the company grows.

Aufgaben

• Administer assessments and screenings per care plan with NP/RN support.
• Prioritize member needs based on assessments.
• Develop and implement care plans within the multidisciplinary team.
• Participate in care rounds to coordinate member experience.
• Learn and utilize Tuesday Health electronic systems for coordinated care.
• Build trusting relationships with members through empathetic engagement.
• Serve as quarterback of the care team to transform care for seriously ill members.
