Job summary

Tuesday Health is seeking an experienced LPN in Ohio to join the Complex Care Navigator team. You will work with nurse practitioners and RNs to perform clinical assessments, participate in care rounds, and help execute individualized care plans, ensuring coordinated, compassionate care for members with serious illnesses.

The role emphasizes trust, empathy, and consistent member engagement, with remote work options and occasional on-site sessions to foster collaboration and team-building.

Qualifications

• Active and unrestricted LPN license in Ohio.
• Experience in clinical/medical settings preferred.
• Experience in a multi-disciplinary care model is a plus.

Responsibilities

• Administer assessments and screenings with support from NP/ RN.
• Prioritize patient needs and contribute to care plans.
• Participate in rounds to coordinate member experience.
• Learn and use Tuesday Health electronic systems to support care.
• Build trust and maintain empathetic member engagement.
• Serve as quarterback of the multidisciplinary team.
