Job summary

The Coordinating Center is seeking a full-time Community Services Coordinator to provide targeted case management for individuals with intellectual/developmental disabilities. This telework role requires visiting clients in the Baltimore City/County region and coordinating medical, social, and supportive services.

You will develop person-centered plans, liaise with providers, and ensure timely access to benefits and community resources.

Qualifications

• Bachelor's or Associate in human services with relevant experience required.
• Experience coordinating services for Medicaid or disability populations preferred.
• Strong communication, advocacy, and problem-solving skills.

Responsibilities

• Coordinate targeted case management for individuals with intellectual/developmental disabilities.
• Assist clients in locating and accessing medical and community services.
• Develop and monitor Person Centered Plans per COMAR regulations and agency policies.
• Maintain caseload of 30–40 clients and support self-advocacy.
• Document services to meet LTSS billing requirements and agency standards.
• Participate in required meetings and trainings.
