Service Coordination, Inc. (SCI) in Annapolis, MD is seeking a Service Coordinator to provide case management for individuals with intellectual and developmental disabilities.

You will establish relationships with clients, families, and providers to coordinate services and support planning. The role emphasizes adherence to state standards, development of Person-Centered Plans, and ongoing monitoring of outcomes, with travel to meet clients on occasion.

Qualifications

• Bachelor's degree in a human services field is required or preferred.
• Experience working with people with developmental disabilities is preferred.

Responsibilities

• Provide Case Management services to individuals with intellectual and/or developmental disabilities.
• Establish positive working relationships with individuals, their team and external stakeholders.
• Develop and monitor Person-Centered Plans (PCPs) and ensure services meet state standards.
• Identify and secure ongoing support services and assist individuals with waivers.
• Coordinate resources and services and document progress and outcomes.
• Ensure HIPAA compliance and maintain confidential records.
