Job summary

Service Coordination, Inc. is seeking a dedicated Service Coordinator to provide case management for individuals with intellectual and developmental disabilities in Westminster, MD.

You will establish Person-Centered Plans, coordinate community resources, and advocate for client independence and well-being. The role requires a bachelor-level degree in a human services field, 2+ years of related experience, and travel to client sites.

Qualifications

• Bachelor's degree in a human services field or equivalent required.
• Two years of experience in human services preferred.
• Experience working with people with developmental disabilities is preferred.
• Travel required to meet client needs and coordinate services.

Responsibilities

• Provide case management services to individuals with intellectual or developmental disabilities.
• Establish, monitor, and update Person-Centered Plans (PCPs).
• Coordinate services and resources to meet individual goals and needs.
• Advocate for individuals and educate families on available supports.
• Complete required documentation and follow regulatory requirements.
