Job summary

Service Coordination, Inc. (SCI) seeks a dedicated Service Coordinator to provide case management and advocacy for individuals with intellectual and developmental disabilities in Maryland.

You will build strong relationships with individuals, families, and providers to support meaningful community living. Responsibilities include establishing and monitoring Person-Centered Plans, coordinating services, and ensuring compliance with program requirements while promoting independence and dignity for

Qualifications

• Education in human services or related field is required or two years of relevant experience.
• Experience in case management and advocacy is preferred.
• Familiarity with state-level services and HIPAA/privacy guidelines is desirable.

Responsibilities

• Provide case management services to individuals with intellectual and developmental disabilities.
• Establish positive working relationships with individuals, their teams, and external stakeholders.
• Develop and monitor a Person-Centered Plan (PCP) and ensure ongoing support services.
• Coordinate resources, referrals, and services to meet PCP outcomes.

Education

Bachelor's degree in human services or related field
