Job summary

The Coordinating Center is seeking a Clinical Care Coordinator to provide day-to-day care management under the REM Program (Rare and Expensive Case Management). You will work with Medicaid recipients under 21 with complex medical needs, partnering with families to create individualized care plans in a hybrid telework and community-visit setting.

Qualified candidates hold a Maryland LMSW/LCSW-C or RN license, with at least two years of case management experience and pediatric exposure; CCM

Qualifications

• Active Maryland Social Work license (LMSW or LCSW-C) or Maryland RN license required.
• Two years of case management experience required.
• Pediatric experience preferred.
• Ability to obtain CCM certification within two years of employment.

Responsibilities

• Develop individualized Care Management Plans with client/caregiver input.
• Coordinate medical and home/community-based services for medically complex individuals.
• Research cost-effective care alternatives and alternative resources.
• Lead the care management process including assessment, planning, education, advocacy and evaluation.
• Monitor outcomes and adjust Care Management Plans to maximize health and goals.
• Facilitate communication among providers, insurers and the health care team.
• Coordinate quarterly on-site visits and multidisciplinary team meetings.
• Maintain confidential PHI and document care management activities in electronic databases.

This is a full-time teleworking position with the expectation to be visiting clients in Baltimore City/County in Maryland.

What You Will Do

As a Clinical Care Coordinator, you will provide day-to-day care management and coordination services within the REM Program (Rare and Expensive Case Management). Your clients will be recipients of Medicaid, most of whom are under age 21, and have complex medical needs. Partnering with their family/caregivers, you will work directly with clients on creating an individualized plan of care with goals to help them remain independent in the community setting of their choice. This position is a hybrid of teleworking and client visits in the community setting including client homes, facility settings, school/day programs, and hospitals. This position requires a commitment to and culturally competent approach to Diversity, Equity, Inclusion and Belonging.

As a Clinical Care Coordinator, you will:

• Facilitate the development of individualized client Care Management Plans, ensuring the inclusion of client/caregiver input.
• Locate, coordinate, and navigate medical services and other home and community-based services for medically complex children and adults with disabilities and comorbid conditions inclusive of individuals experiencing financial, housing and resource instability.
• Research and recommend cost effective alternatives to care, as appropriate.
• Engage the case management process and best practices to guide client service delivery to include assessments, documentation, planning, implementation, education, advocacy, and evaluation.
• Evaluate actual outcomes, as well as desired outcomes and adjust Care Management Plan interventions accordingly to maximize health and goal achievement potential.
• Facilitate communication between community-based providers, insurance carriers, the multidisciplinary team and supports required by the client.
• Coordinate quarterly on-site visitations as well as convening and attending multidisciplinary team meetings, and maintaining periodic, regular contact with clients and other members of the health care team.
• Complete timely documentation of care management activities in multiple electronic databases, maintaining confidentiality of PHI in all settings.

The salary for this position is $70,000-$72,000 and comes with all of our full time benefits. This position requires the CCM certification within two years of employment, with a $1500 stipend upon completion.
