Service Coordination, Inc. is seeking a Service Coordinator in Prince Frederick, MD to provide case management services for individuals with intellectual and developmental disabilities. You will establish relationships with clients, their families, and external partners.

The role offers a base salary of $64,395-$79,861, travel opportunities, a quarterly bonus, generous benefits including tuition reimbursement, 403(b) match, paid leave, and employer-provided equipment.

Responsibilities

• Establish positive working relationships with individuals, their teams, and external stakeholders.
• Develop and monitor a Person-Centered Plan (PCP) to identify outcomes and needed support.
• Identify and secure ongoing support services and ensure the quality of services.
• Meet state-mandated requirements for the Service Coordinator duties.
• Coordinate resources and services to achieve PCP outcomes.
• Advocate for individuals and educate families about services.
