Job summary

The Coordinating Center is seeking a full-time Coordinator of Community Services to manage a caseload of 30–40 individuals with developmental disabilities. You will advocate for clients, support self-advocacy, and help develop Person-Centered Plans in accordance with COMAR regulations and agency policy.

You will locate and coordinate medical, social, habilitative, and other supports, ensuring the most integrated settings.

Qualifications

• Bachelor’s degree in a human service field; or an Associate degree with two years of human service experience; or seven years of relevant work experience.
• Experience coordinating services for Medicaid/waiver or IDD populations strongly preferred.
• Competency in negotiation, advocacy, and system navigation.

Responsibilities

• Maintain a caseload of 30–40 clients with intellectual/developmental disabilities.
• Advocate for clients and support self-advocacy in service planning.
• Develop Person-Centered Plans in line with COMAR and policy.
• Research and coordinate services to meet clients' needs in the most integrated settings.
• Document services to meet LTSS billing and internal requirements.
• Attend required meetings and maintain professional relationships with providers.
