Job summary

The Coordinating Center is seeking a full-time Coordinator of Community Services to manage a caseload of 30–40 clients with intellectual/developmental disabilities. You will coordinate medical and community services, advocate for clients, and develop Person-Centered Plans in line with COMAR and agency policy.

You will visit clients in settings where supports are provided, maintain thorough documentation, and participate in required meetings to ensure coordinated, client-focused care.

Qualifications

• Bachelor’s degree or equivalent experience in human services.
• Experience coordinating services for Medicaid/waiver or IDD preferred.
• Competency in advocacy, problem solving, and family-centered care.

Responsibilities

• Maintain a caseload of 30–40 clients with IDD.
• Develop and implement person-centered plans per COMAR and policy.
• Research and access services to meet client needs in integrated settings.
• Document client services to meet billing standards.
