Job summary

Qualifications

• Three years of experience in patient care, care management, transition/discharge planning, medical assistance, healthcare coaching, or patient care coordination and certification or degree in healthcare.
• Excellent interpersonal and communication skills.
• Ability to adapt quickly as needs arise.
• Knowledge of available health resources.

Responsibilities

• Provide operational support and transition duties for high risk complex patients.
• Collaborate with care management team and other providers to deliver patient-centered services.
• Manage referrals, pre-screening, scheduling, and initial assessments.
• Conduct patient follow-up calls and reinforce education and self-management actions.
• Lead interdisciplinary care conferences and document in the medical record.
