Qualifications

• Three years of experience in patient care, care management, transition/discharge planning, or related fields.
• Licenses or certifications in a healthcare field as applicable.
• Strong written and verbal communication with patients and teams.

Responsibilities

• Referral management: screen and monitor referrals and maintain caseload records.
• Patient consultation: contact new patients and schedule initial assessments.
• Interventions: follow-up calls to reinforce education and care planning actions.
• Transition management: ensure effective transitions between care settings.
• Documentation: accurate medical record documentation compliant with guidelines.
• Lead interdisciplinary care conferences and coordinate with providers.
