Job summary

Qualifications

• Three years of experience in patient care, care management, transition/discharge planning, medical assistance, healthcare coaching, or patient care coordination.
• Excellent interpersonal and communication skills.
• Ability to adapt quickly as needs arise.
• Knowledge of available health resources.

Responsibilities

• Referral Management: Screen, monitors pending and prioritized referrals; maintain a record of daily caseload and workload.
• Patient Consultation: Contact new patients by phone to introduce the program, obtain verbal consent, and schedule initial assessment.
• Interventions: Conduct patient follow-ups, reinforce education and care planning actions; collaborate with providers for integrated care plans.
• Transition management: Manage designated transition duties to ensure seamless care transitions; participate in process improvement initiatives.
