Job summary

UnitedHealthcare is seeking a Care Coordinator to serve as primary manager for a panel of members with chronic and complex health needs. You will support the team with clinical and non-clinical activities in a person-centered care coordination approach.

The role emphasizes coordinating medical, behavioral, and social needs, collaborating with providers, and promoting access to HCBS and community resources. Idaho Falls area, remote options available.

Qualifications

• 1+ years of experience with Medicaid/Medicare or social determinants of health
• Ability to use MS Office (Word, Excel, Outlook)
• Access to a dedicated home workspace and secure PHI handling
• Reside where high-speed internet is available and able to travel up to 50%

Responsibilities

• Serve as primary care manager for dual eligible members
• Conduct comprehensive needs and wellness assessments
• Collaborate with internal care team and community resources
• Assist with HCBS supports and referrals as needed
• Coordinate transitions from ER, hospital, or SNF
• Educate and coach members for self-management of care
