Qualifications

• Three years of experience in patient care, care management, transition/discharge planning, medical assistance, healthcare coaching, or patient care coordination - and - Certification, Associate Degree, or Bachelor's degree in a healthcare field.
• Excellent interpersonal and communication skills.
• Ability to adapt quickly as needs arise.
• Knowledge of available health resources.

Responsibilities

• Screen, monitors pending and prioritized referrals and maintains a record of the daily caseload.
• Promptly contacts new patients by phone to introduce the program and schedule the initial assessment.
• Follow-up with patients to reinforce education, self-management, and care planning actions.
• Collaborates with healthcare providers across the continuum to ensure integrated care planning.
• Advocates for patients by coordinating with payers, physicians, and community services to support transition of care.
