Responsabilidades

• Referral Management: Screen, Monitor pending and prioritized referrals.
• Patient Consultation: Contact new patients by phone to introduce the program and schedule the initial assessment.
• Interventions: Follow-up calls to reinforce education, self-management, and care planning actions.
• Transition management: Ensure effective transitions of care between settings and support process improvements.
• Documentation: Timely and accurate documentation in the medical record per guidelines and billing/reimbursement standards.
• Lead interdisciplinary care conferences and team huddles to promote collaborative care planning.
