Job summary

Intermountain Health is seeking a Remote Care Management Specialist to support high-risk patients, coordinating referrals, pre-screening, scheduling and transition management. You will work with care teams, patients, families, providers, and payers to deliver efficient, patient-centered services.

This full-time role requires daily collaboration, in-person meetings as needed, and strong HIPAA/compliance practices. Remote work is available with occasional onsite Nevada meetings.

Qualifications

• Three years of experience in patient care, care management, transition/discharge planning, medical assistance, healthcare coaching, or patient care coordination.
• Excellent interpersonal and communication skills.
• Ability to adapt quickly as needs arise.
• Knowledge of available health resources.

Responsibilities

• Screen, monitors pending and prioritized referrals and maintains workload records.
• Contact new patients to introduce the program and schedule initial assessments.
• Conduct follow-up calls to reinforce education and self-management.
• Collaborate with care providers to ensure integrated care plans.
• Lead interdisciplinary conferences and maintain documentation in the medical record.
• Ensure productivity standards and contribute to standard work development.
