Qualifications

• Three years of experience in patient care, care management, transition/discharge planning, medical assistance, healthcare coaching, or patient care coordination.
• Excellent interpersonal and communication skills.
• Ability to adapt quickly as needs arise.
• Knowledge of available health resources.
• Discharge/transition planning, healthcare coach, health advocate, or medical assistant experience.

Responsibilities

• Referral management for pending and prioritized referrals.
• Patient scheduling, initial assessment, and pre-screening.
• Patient consultation and education to support care plans.
• Care coordination and follow-up to reinforce self-management.
• Documentation and collaboration with the care team and providers.
