Job summary

Intermountain Health in Las Vegas, NV seeks a Care Management Specialist to support high-risk patients through referral management, care planning, and transition coordination. This role requires HIPAA compliance, strong communication, and collaboration with care teams, patients, and providers.

Qualifications

• Three years of experience in patient care, care management, transition/discharge planning, medical assistance, healthcare coaching, or patient care coordination - and - Certification, Associate Degree, or Bachelor's degree in a healthcare field.
• Excellent interpersonal and communication skills.
• Ability to adapt quickly as needs arise.
• Knowledge of available health resources.

Responsibilities

• Promotes HIPAA compliance and maintains patient information confidentiality.
• Screen, monitors pending referrals and manages workload daily.
• Contact new patients by phone to introduce the program and schedule assessments.
• Follow-up with patients to reinforce education and care planning actions.
• Collaborate with providers, payers, and community services to create integrated care plans.
• Lead interdisciplinary care conferences and ensure effective communication.
• Document in the medical record per guidelines and billing standards.
• Escalate concerns promptly when issues arise.
• Assist in transitions of care between settings and participate in process improvements.
